Patient Registration

Robert Street Surgery

SMS Messaging

The practice is currently sending text message to patients for varying reasons.

Please ensure that we have your current mobile telephone number in order to do this.  If you do not wish to receive any texts from the practice please let us know in writing and we can organise that for you.

The Practice Partnership

Collectively the doctors are a medical partnership with all patients registered to the Partnership rather than a specific practitioner.

The partnership is contracted to provide a variety of services by the Hywel Dda Health Board, part of NHS Wales.

Register As A Patient

Anyone wishing to register with the Practice should contact reception to introduce themselves as a new patient.

All patients are registered to the Practice rather than a specific practitioner, however appointments can be booked with the doctor of choice.

Please inform the Practice if you have ever been a member of the Armed Forces or if you are currently a carer when registering.

We are currently not accepting new registrations, so a patient wishing to register should complete a registration form and a surgery will be allocated to you.

Acceptance will depend on your address and whether it falls within our practice boundary.

As soon as possible after registration a medical record is set up.  Past records can then be called from the previous GP practice.

All data is treated as highly confidential and can be accessed only by the medical staff and employees of the Practice.

Temporary Patients

You are able to see a doctor anywhere in the UK, if you are away from home and in need of medical help.  You can do this by simply asking to see the nearest doctor, as a temporary resident.  We will be happy to see any relatives or friends staying with you, if they need a doctor.  Please note temporary patients are on an allocation basis with another surgery in Milford Haven.

Chaperone Policy

As a Practice we are committed to providing a safe, comfortable environment for our patients.  If you would like a chaperone during your consultation, please ask the doctor.  A Practice Nurse or Health Care Assistant will usually act as your chaperone, but on rare occasions a Receptionist may be asked to chaperone.

Contact Details

Please make every effort to ensure that your name, address and telephone/mobile numbers are kept correct.
